Filling Out The Game Card, for Referees and Assistant Referees

Before the Game Starts: 

  • First, get the card of the team on your side of the field from the coach.
  • "Region," "Div:," "Team #," "Team name," "Team Colors," "Coaches' names," "players' names," "numbers" (numerically) should already be filled in by the Coach. If not, have coach fill it all in.
  • ALL players should be on the card, regardless of whether or not they will be showing up.
  • Ask if anyone is missing or will not be playing and why.
  • If a player who is listed on the Game Card is unable to play the entire game, draw a single line through all 4 substitution boxes next to their name and write in the reason why next to their name (such as “injured”, “ill”, “absent”, etc).
  • Fill in the Date, Game Time, Field Number.
  •  BEFORE THE GAME STARTS : on the home team game card, have the field monitor sign name in the "Additional Comments" area on the back.

Note that for Extra League matches, the away team might have a game card with a different format. All the information on any card is listed somewhere, back or front, top or bottom, etc.

Before and During Game: Indicating Player Substitutes on the Game Cards

  • The Assistant Referee is to indicate only those players who are NOT playing (because there are fewer of them) at the beginning of each period by placing a mark (preferably an “X”) in the 1st, 2nd, 3rd or 4th substitution box by the player’s name.
  • The Assistant Referee shall indicate the goalkeeper of each period with a "G" (NOT "K", because it looks too much like "X") in the appropriate box in the 1st, 2nd, 3rd or 4th substitution box by the player’s name. The lower ages have restrictions on number of periods, so this needs to be recorded.
  • If a player started a “quarter”, but has to be substituted out during that quarter due to injury, illness or some other reason, do not mark them out for that quarter since you have already marked another player out. Leave the game card as is.
  • If a player who is listed on the Game Card started the game but cannot complete the game, draw a single line through the substitution boxes that apply next to their name and write in the reason why next to their name (such as “injured”, “ill”, “absent”, etc). (Example: player injured before halftime and cannot play any more, put a line through the "3" and "4" boxes, and note the reason next to the player's name.) This is important, because otherwise someone might call that coach and ask why a player didn't play.
  • The first two columns of boxes on the Game Card to the right of the players' names are used to record which half a player scored a goal (1st column for 1st half, 2nd column for 2nd half). Make "Tally Marks."

At Halftime

  • The referee team meets in the center and records the halftime score, YOUR CARD'S TEAM'S SCORE FIRST! And, note the team # who is leading, if either. If tied, draw a line through the "In favor of team #" spot.

Note that for Extra League matches, the teams do not have team numbers like our core teams. They are from a different region, so put the region's name in the halftime score box if ahead. "TO" for home. "Ventura," for example, if away.

At the End of the Match

  • The referee team meets in the center and records the final score, YOUR CARD'S TEAM'S SCORE FIRST! And, note who won and who lost. If tied, draw a line through the "Winning Tm #" and "Losing Tm #" spots.
  • Note that for Extra League matches, the teams do not have team numbers. They are from a different Region, so put the region's name in the "Winning TM #" and "Losing Tm #" lines. "TO" for home. "Ventura," for example, if away, such that each team can be identified.
  • Then, sign (PRINT) your name on the cards and fill in the team you are volunteering for in the "Tm #" spot, legibly, to get team points.
  • Please report successes or problems from your match or field to the Referee Tent on the field. If you remember something afterwards, please send an email to Sean Donohoe 
  • If there was any Disciplinary Action Taken, the center referee will fill that out, and might ask Assistant Referees for additional information.
  • If you and the center referee want to fill in the "Conduct of:" section of the game card, feel free to do so
  • Bring the game cards to the Referee Tent.  On Sundays, the ref tent should be up near "The Tree" between Fields 17 and 18. There may not be a ref tent. If so, Center Referee should hold the game cards and contact a Regional Board member to ask what to do with them. (Hopefully, one will be on site.)
