Be sure to include your Team # (e.g. 10UG05) and the coach's full name.
Describe the issue (e.g. missing uniform or uniform item; team uniforms; etc.) and the uniform color (e.g. POWB / BLK / WHT)
Parents, direct all questions through your coach or team manager.
Please do not contact the uniform vendor directly.
Player uniforms for U6 – U14 Boys and Girls teams will be distributed on Sunday August 29th, 2021
Location: at the Thousand Oaks Teen Center parking lot (across the street from the soccer fields)Times:

Procedure:
One team representative from each team (coach, assistant coach, team manager) should show up at the assigned time (or better yet, 15 minutes early) for their team’s Division. Uniforms for an individual player will NOT be distributed to that player’s parent or guardian at this distribution.When you arrive, please go to the designated area for your division. Signs will be posted to along the
fence for each division (6UB, 6UG, etc.). The Division Administrators (DAs) will be there handing out
uniforms to each team.Uniforms will be distributed within each Division and Group, in chronological order, based on when a team’s Team Volunteer Information was submitted (see the Team Volunteers 2021 web page). If your team has not submitted their Team Volunteer Information by Thursday August 26th at 6pm, you will not be able to pick up your team’s uniforms at this time.Your team representative also needs to turn in the Team Sponsorship form along with a sponsor check in the amount of $100, payable to AYSO Region 9.
The sponsorship form will be the only paperwork that will be collected. Do not show up with your volunteer information in hand. It is imperative that you show up on time. Although no uniforms will be handed out before 10:00AM, if your team number is called and you are not there, you will be passed over and moved to the bottom of the list; no exceptions.
For those coaches that have two teams, you will need a representative for each of your teams (an Assistant Coach, Ref, Team Parent etc.). The same goes for anyone who is unable to attend. In the rare case you miss the deadline to enter in your volunteer information, we will be having another distribution date, but no details are available at this point.
